Important Information to Know Before You Visit
A resident who is suspected or confirmed to be infected with COVID-19, or quarantined for a COVID-19 exposure, cannot have visitors.
A resident who has been diagnosed with COVID-19/tested positive is allowed visitation only after they have met the criteria for discontinuation of isolation as defined in guidance from the NJ DOH and CDC.
A resident who is in isolation because, within the past 14 days, they were either admitted/re-admitted, left the facility for a procedure/medical appointment, or were exposed to a COVID-19 positive employee, is allowed visitation only after they have met the criteria for discontinuation of isolation as defined in guidance from the NJ DOH and CDC.
If you are unsure if your loved one is among the residents currently under isolation precautions and, therefore, unable to have in-person visitation, please contact us.
The resident has a right to choose visitors and make preferences. Each resident will be consulted to determine who the resident would wish to visit with in person.
If you are unable to visit in person, you may still visit virtually through Skype. Skype sessions may be scheduled below.
Use the restroom. Visitors will not be permitted to use the restrooms in the facility.
When You Visit
No more than two people may visit at a time.
Put on your face mask and enter the facility, where you will be screened by our staff.
Present your Consent for Limited Visitation During COVID Outbreak form and Visitor Wellness Questionnaire. Blank forms will be available if you haven’t completed them in advance.
Staff will take your temperature.
Perform hand hygiene.
If a visitor has any COVID-19 symptoms, they will not be permitted to visit with a resident.
Visitors are not allowed past the screening area. After you are cleared to visit, return to your car, drive around to the designated visitation area and park your car.
The visitation areas are set up to ensure social distancing:
— The benches are for visitors. — The resident will be brought down to the area and positioned by staff. — Please do not move the benches.
During Your Visit
Maintain the distance we have set.
Bridgeway or Avalon staff will remain near the resident.
The resident, staff and visitors must keep their masks on the entire time.
There is no hugging, kissing, handshakes, touching, handing of gifts/items.
Food and drinks are not permitted.
Be considerate and leave at the end of the allotted time. We have to disinfect the seating surfaces prior to the next visit.
After Your Visit
If you test positive for COVID-19 or have symptoms of COVID-19 within 14 days of visiting, notify the facility immediately.
Strict adherence to these procedures is necessary to protect everyone’s safety. Failure to follow it may result in restriction of future in-person visitation. Please understand that while these measures may seem extreme, they are based on the guidelines from the NJ DOH.
