Bridgeway Sub-Acute Rehabilitation in New Jersey
The Bridgeway Care and Rehabilitation interdisciplinary team works with individuals recovering from heart disease or heart surgery to help them reach their goals in the sub-acute rehabilitation setting. Physicians, therapists, nurses, aides, social workers and dieticians work together to create and implement an individualized, progressive cardiac program for our patients. We focus on helping our patients regain their strength, mobility and endurance whileminimizing pain so they can return home and resume their daily routine.
At our Bridgeway Care and Rehabilitation Centers, we have implemented a cardiac program to help our patients avoid unnecessary re-hospitalization for their heart-related conditions. This outcome-based program specifically targets patients with the diagnoses of CHF, Ischemic Heart Disease, Hypertension, Atrial Fibrillation, CABG and/or Aortic Valve Replacement or Repair. We closely monitor our patients for symptoms of heart failure throughout their rehabilitation stay.
